Referral Instructions for LEAP (Internal and External Referrals)

When submitting a referral for an ISO request, please include the following details. Our Care Team will review each referral and determine which provider or groups of providers can best support the client’s needs.

  1. Introduction and Reason for Referral: Briefly explain the individual’s background and the reason for the referral. This helps our Care Team understand their unique situation and determine the best course of action.

  2. Specific Request: Clearly state what the client needs (e.g., an initial consultation, additional services, or questions about fees or insurance). This will ensure the referral is directed to the appropriate resource.

  3. Availability and Communication Preferences: Provide the client’s availability and preferred method of contact (phone, email, or text). This allows us to follow up in the way that works best for them.

Once the referral is received, our Care Team will assess the details and match the client with the clinician who is best positioned to provide the needed support.
